Zombie heads were available on 31 October 2005 and a week after it, during the 2005 Hallowe'en event. During this time, zombies were placed in Lumbridge Swamp and kids were placed in Lumbridge. If a player right-clicked on the zombies, they could talk to them or insult them. When a player insulted them, depending on the zombie, they either split in half from top to bottom, split along their waist, or their heads would fall off and the player had to catch it. Scaring a zombie also gave players the 'scared' emote. When they caught the head, it appeared in their inventory.
Players can right-click the head and talk with it or select actions which other players can see. The head cannot be equipped. If a player lost the head, they could go back to Lumbridge Swamp and dig with a spade to find another head. Players could scare the 'trick or treating' kids in Lumbridge with the zombie head, making them drop some sweets. Players could also treat the kids with money or stolen sweets. The candy was nontradeable and came in different colours: red, blue, green, and purple. The sweets melted into chocolate dust after a week.
Any player owning a Zombie Head has access to two unique emotes provided he has the head. The player may prop the head on his/her hand and say, 'Alas!', or the player may hold the head by the hair and let out an 'evil laugh'.

- The "Alas!" and the examine text (Alas... I hardly knew him.) is a reference to the famous Shakesperean tragedy Hamlet. In one scene, Hamlet finds the remains of a friend of his and performs a soliloquy with the skull, one line being the famous (and often misquoted) "Alas, poor Yorick! I knew him, Horatio."
- When you talk to the head, you find out its name is Ed Cranium, "ed" is a slang term for head and the cranium is the medical term for the head, so the head's name is basically "head head". His name may also be a reference to Eddie the Head.
- If you drop a zombie head and get another one and try pick up the dropped zombie head it says "The head bites at you! Good job you already have one."
- There is currently a glitch that allows the owner of the zombie head to temporarily glide along the lands of RuneScape. If you click to the area you wish to travel to, then quickly press any of the two unique emotes you will glide momentarily while exclaiming the chosen words brought with the emote (ex. Alas!)
- If a player has dropped their zombie head, another may be obtained by using a spade to dig anywhere within the Lumbridge Swamp.
- There is a unique dialogue that takes place by using the zombie head on Dr. Fenkenstein during the Creature of Fenkenstein quest.
