The Truth serum is an item obtained during The Hand in the Sand quest. Players make it by using a rose-tinted lens on the unfinished truth serum potion on Betty's counter.
During the While Guthix Sleeps quest, a truth serum can be obtain by talking with Betty. It is used with Enriched snapdragon to make Super truth serum.
