The infernal greathammer is a cosmetic override from the Battle of Lumbridge. It can be obtained by combining 1,000 sacred metal fragments with an infernal greathammer head.
Note: Despite being labelled as a greathammer override, this skin can only be used with mauls - not warhammers, as the name may suggest. It does, however, work with Balmung, the Gadderhammer, the Blisterwood polearm, Barrelchest anchor and Exquisite halberd. It also works with a Banner of Zamorak.
