The Enhanced Excalibur is an upgraded version of Excalibur and can be obtained after completing the Seers' Village Diary. The Lady of the Lake will enhance Excalibur after completing the hard portion of the Seers' Village Diary and wearing the Seers' headband 3. Enhanced Excalibur's stats are comparable to that of a rune longsword, and it has an enhanced special attack that, in addition to a better boost in defence (15% + 2 boost), heals the player 4 hp every 2 seconds for 10 seconds for a total of 20 hp. This uses up 100% of the special attack bar.
Once enhanced, it can no longer be mounted onto the wall in a player's Quest Hall.

- Healing at the bank when damaged.
- Healing when training Thieving or Agility.
- Healing when training Ranged (especially at Aviansies) or Magic.
- Defence boost in combat minigames like Soul Wars (especially when using Ranged or Magic)
- Healing during the Fight Caves while giving a Defence bonus.
- Healing during trips to the Abyss.
- Healing when doing Phoenix Lair.
- Healing when doing long trips at Barrows.
- Camping at certain areas without having to bank for food.
- Replacement for defence potions and emergency food during Slayer tasks.
- Replacement for defence potions and emergency food when training on creatures who do just enough damage to warrant a Bunyip.
- Healing when in Barbarian Assault, can be used when healers are occupied elsewhere.
- Healing when fighting at Turoth and Kurask seeing as the weapons to kill them do not use Special Attack energy.
NOTE: If you happen to log out during the special's effect, the special will NOT continue healing you after you log in again. However, the effect will continue if you switch weapons.
Like poison damage, the effect will stop when a player is banking, and continue immediately when the bank is closed.

- After the Game Engine update of 2 September 2009, the special attack didn't activate until you attacked something. This was fixed later the same day.
- The special attack can be used while resting - the player will stand to perform the special attack emote, but resume sitting afterward.
- Some players have reported a glitch where the Enhanced Excalibur will perform its special attack randomly. The sword would remain within their inventory and will perform the special attack, but without the player having to click on Special Attack. This sword does not have a right-click option in the inventory menu to activate its special.
