The Dragon two-handed sword is the second strongest two-handed sword available, succeeded only by the various types of Godswords, and the fourth strongest non-degradable weapon, surpassed only by the Barrelchest Anchor, the Godswords and Balmung. It can only be wielded by members who have at least 60 Attack. It is dropped by the Chaos Elemental and the Kalphite Queen.
The Dragon Two-Handed Sword has a special attack, Powerstab, which can hit up to 14 enemies in the squares around the player, including the diagonal squares,but only in a multicombat area, thus making the special attack effective in the Pest Control minigame and other fights with multiple opponents such as in the Castle Wars minigame. The Powerstab can hit just as high/often as the normal attack. However, it uses 60% of a player's special attack energy, so a player can't do it in rapid succession. Players typically prefer to buy an Abyssal Whip or Saradomin Sword in place of the dragon 2h sword. Against players the maximum hit for the dragon 2h sword is 51 if using PvP armour with all the best other equipment, potions, prayers etc, or 49 with void and the same.
With the release of Item Lending, the Dragon Two-Handed Sword, like the Godsword, is in somewhat popular demand for inexperienced players wishing to see their maximum hit and/or to see if it is something they wish to buy. This weapon is mostly seen borrowed/lent near the Pest Control landers.
As with all dragon equipment (except the dragon platebody and the Dragon sq shield), this item cannot be made using the Smithing skill. However, the Dragon 2h sword is one of the few dragon weapons that do not require the completion of a quest. Because of this and its large strength/slash bonus it is popular in bounty (+1) and for training in dense areas.
Recently, in May 2009, the price of the Dragon 2h Sword crashed. It fell from around one million coins to around 600,000 coins. After a slight price recovery around 19 May 2009, the Dragon 2h continued to fall in price and in the crash people were frantically selling them in junk trades. The price has made a significant recovery, and has settled at around 1.5 million.

- Prior to its release, the dragon two-handed sword was shown in the old RuneScape banner. During the time it was originally shown on there, there was a dragon pickaxe shown next to the gold ore rock in the banner as well. However, after the release of this sword, it was removed from the banner.
- Before the giants received a graphic update, Fire giants appeared to be wielding dragon two-handed swords. This was brought up in an edition of Postbag from the Hedge.
- The Dragon 2h now resembles a Dragon longsword. This has caused a lot of outrage from many players who bought a Dragon 2h before the update. Also, the player runs with it in a strange way. It is unknown if this is a glitch. Many players disliked the new Dragon 2h look and sold their Dragon 2h swords after the update.
