The Dragon ceremonial hat is a possible prize from the Squeal of Fortune on the rare slots. It gives no bonuses and is purely cosmetic. The hat is part of the Dragon ceremonial clothes set. This set along with the Queen's Guard outfit can only be won "throughout the summer months" according to Jagex.
One of the unique features of this item is that, when the entire set is complete, it can be re-coloured when you wear all 5 items by right clicking. This ability is also shared with the Dragon Ceremonial clothes. "Re-colouring is a first for cosmetic items of this nature" according to Jagex.
