This Decapitated head is an item used only in Creature of Fenkenstrain quest. It is one of the five parts of the monster that Dr. Fenkenstrain is creating. Players can find it by digging a grave where the Gardener Ghost leads.
The head was actually the Gardener Ghost's head, until Dr. Fenkenstrain murdered him via decapitation. Players fill the head with a pickled brain, which originally belonged to Lord Rologarth.
