A Bowl of milk, honey and guam is used during the Meeting History quest. It is used as a medicine for Baby Sarah to cure her illness.
The item is obtained by using a special bowl with the beehive in the family's backyard, creating a bowl of honey. A bucket of milk is then added, making a bowl of milk and honey. Finally, by grinding a guam leaf with a pestle and mortar, players can obtain ground guam. When added to the bowl, players will obtain the cure to Sarah's illness.
After curing his daughter, Roger happily narrates a story about how humans arrived on RuneScape through the Portal of Life.

- The grown-up Sarah informs that she has a cough since she was a baby. This mixture of milk, honey and guam is probably a traditional method of preparing cough syrup.
